We mostly accept all insurances have some limitations, please note the following:

• For Workers Compensation or Personal Injury Protection (No Fault) insurances, you will need your claim number or WCB number.
• For Department of Labor or Federal Worker Compensation Insurances, you will need a formal written authorization from your department to see a Neurologist for new patient consultation.
• For Commercial Insurances, you will need to provide your complete insurance information details via email and our staff will verify your insurance prior to scheduling. This process can take up to 2 weeks. Also please note, we take some insurances in certain office locations and not in others. We apologize in advance and submit that we are at the mercy of insurance companies regarding service locations and credentialing and it is not in our control.
